At my practice, I prescribe mineral therapy such as the Active Elements and Ionic minerals from Biotrace. If you'd like to assess your current mineral status - this online questionnaire is a great way to do this. 

About the online appraisal

To complete the appraisal, please login or register. Registering allows you to keep track of your assessments over time and ensures your privacy. Once logged in, you can access the questionnaire.

At the very end of the questionnaire, you'll have the opportunity to enter any comments that you feel may be important or relevant to your case (the more you can tell us about yourself the better). Maybe give a bit of thought to these issues and make a few notes so that you're ready before you go online.

The questionnaire will take you about 15 minutes to complete.

Report and recommendations

We will send you an email with your report and recommendations. You'll receive this within a few days of completing the online appraisal.
